La transformation actuelle du système de santé marocain renforce la nécessité de disposer d’outils de pilotage capables d’articuler ressources, activité et performance. Cet article analyse la place du contrôle de gestion dans les établissements hospitaliers publics et examine les conditions de son intégration dans la nouvelle gouvernance territoriale portée par les Groupements Sanitaires Territoriaux (GST). L’étude s’appuie sur la phase qualitative d’une recherche doctorale menée dans la région Béni Mellal-Khénifra auprès de dix responsables hospitaliers. Une analyse thématique assistée par NVivo permet d’identifier quatre dimensions structurantes : la gestion budgétaire, les tableaux de bord, le reporting et le système d’information. Les résultats montrent que ces instruments ne produisent pleinement leurs effets que lorsqu’ils sont articulés dans une même boucle de pilotage reliant programmation, mesure, analyse des écarts, dialogue de gestion et actions correctives. La confrontation de ces résultats au décret n° 2-22-580 du 3 mars 2023 met en évidence une convergence entre les pratiques observées et l’architecture institutionnelle du contrôle de gestion public. L’article propose ainsi un modèle de pilotage à deux niveaux, établissement et GST, fondé sur la fiabilité des données, la cohérence des indicateurs et la responsabilisation des acteurs. Il conclut que l’institutionnalisation du contrôle de gestion hospitalier ne doit pas se limiter à la production de tableaux et de rapports, mais doit soutenir une véritable capacité d’arbitrage et d’apprentissage organisationnel.

Abstract

The ongoing transformation of the Moroccan health system increases the need for steering mechanisms capable of linking resources, activity and performance. This article examines the role of management control in public hospitals and the conditions for its integration into the new territorial governance framework embodied by Territorial Health Groups (GSTs). The study draws on the qualitative phase of doctoral research conducted in the Béni Mellal-Khénifra region with ten hospital managers and professionals. NVivo-assisted thematic analysis identifies four structuring dimensions: budgeting, dashboards, reporting and information systems. The findings show that these instruments become fully effective when they are integrated into a single steering loop connecting planning, measurement, variance analysis, management dialogue and corrective action. Comparison with Decree No. 2-22-580 of 3 March 2023 reveals substantial convergence between observed practices and the institutional architecture of public-sector management control. The article therefore proposes a two-level steering model linking hospitals and GSTs, based on reliable data, coherent indicators and managerial accountability. It concludes that the institutionalization of hospital management control should not be reduced to the production of dashboards and reports, but should support genuine arbitration, coordination and organizational learning.
